Cat Surrender Form

Tell us as much as you can. You do not need to vaccinate, sterilize, test, groom, or take the cat to a veterinarian before applying unless Gato Rei Sanctuary specifically asks you to do so.

Before completing the form

Submitting the PDF does not transfer ownership or guarantee admission. A surrender becomes final only after the Sanctuary approves the intake, physically accepts the cat, and an authorized representative signs the acceptance section.
